Hi Dan

Spoken like a true pro who's been there! I'm guessing you are one VP who's knocked heads together and of course being a VP of sales AND marketing you can alwys have conferences with yourself!

I think it's also the case that sales want's to take much of the credit for company success and so is somewhat dismissive of marketings role - I've seen this myself several times. The nature of the beast means they tend to like to operate very independently and since they are held to very tangible targets it's always been a more solitary function.

Blinding disconnect is right, and the blame goes to the VPs responsible for the function. If the guys responsible for execution are not in synch, the whole operation goes out of whack, and frankly the CEO is the one with the authority to knock their heads together, so he/she is to blame as well.
